This occurs when permeability to albumin is increased, but without a marked increase in the
filtration of other higher molecular weight proteins (immunoglobulins, ?2-macroglobulin).
Microalbuminuria, the pathological excretion of urinary albumin at a ratio more than 30mg / per
gram of urine creatinine, is widely recognized as a useful marker of the development and
progression of renal disease and generalized vascular endothelial dysfunction. Historically,
microalbuminuria has been thought to herald the onset of altered glomerular structure and
function.
Numerous clinical studies in non-diabetic populations have found an association between
microalbuminuria and cardiovascular risk factors, target organ damage and the presence of
cardiovascular disease. Microalbuminuria is predictive of cardiovascular mortality in both
diabetic and nondiabetic populations. The presence of microalbuminuria indicates a widespread
disturbance of endothelial function, resulting in an enhanced risk for the development of
atherosclerosis. It may thus serve as a useful biomarker for systemic vascular disease and is a
very useful and important screening technique for CVD.
